As agreed, please find attached a draft evaluation questionnaire for the STAR training materials (for use by DPAs)

If you get comments to me today, then I can make changes to this tomorrow morning (Friday) - I'm off on leave the following week.

In particular, is there anything missing that we want to know about? Anything we're worried about or feel we'll need to show for the evaluation?

This survey is for DPAs, we can create a separate one for DPOs, which would be mostly similar, or add a question asking respondents to identify themselves as DPA/DPO/other. Its mostly the same from the analytical back end.
We could add a final question asking DPAs to provide contact details if they would want to participate in trials. But given the rest of the survey is anonymous, leaving this off avoids personal data concerns, we should have contact details for people we've emailed anyway.
