Maybe a separate question, focusing on the usefulness of the comment section could be added
At q11 the possibility of multiple answers could be mentioned
If the remaining questions are the same a first question could be added to distinguish between dpas and dpos
Contact details: it was part of the WP2 questionnaire (whether they want to participate in further stages of the project). I would leave this question out

As agreed, please find attached a draft evaluation questionnaire for the STAR training materials (for use by DPAs)

In particular, is there anything missing that we want to know about? Anything we're worried about or feel we'll need to show for the evaluation?

The questions currently take into account the requirements in D2.4

This survey is for DPAs, we can create a separate one for DPOs, which would be mostly similar, or add a question asking respondents to identify themselves as DPA/DPO/other. Its mostly the same from the analytical back end.
We could add a final question asking DPAs to provide contact details if they would want to participate in trials. But given the rest of the survey is anonymous, leaving this off avoids personal data concerns, we should have contact details for people we've emailed anyway.
